By submitting your agreement, you are committing to participate in the one-year program. As a parent (guardian), you are consenting to your mentee’s attendance at all mandatory program activities, which include workshop sessions, orientations, community services, and program-related functions. Additionally, you agree to support the mentee in maintaining weekly communication with their mentor.
